What is an FAA Class I Medical Exam?

A Requirement to Fly

The FAA Class I Medical Exam is the most comprehensive of the three certification classes and is designed to ensure that pilots are physically and mentally fit to operate commercial aircraft safely. A First-Class Medical Certificate is required for:

  • Airline Transport Pilots (ATP) who operate commercial airliners

  • Corporate and charter pilots flying under Part 121 or Part 135 operations in ATP roles


While some pilots may not need a Class I certification for their current duties, many choose to obtain or renew it proactively to keep career options open or to stay current with the highest level of medical clearance available.

What to Expect During Your FAA Class I Medical Exam

The FAA Class I Medical Exam is designed to identify any medical conditions that could impair a commercial pilot’s ability to operate an aircraft safely, such as cardiovascular disease, epilepsy, diabetes requiring insulin, or psychiatric disorders. 

 

During your appointment, we will:

Review Your Medical History

You’ll complete FAA Form 8500-8, which includes questions about your medical history. Please bring a list of any medications you’re currently taking and details about previous medical treatments, if applicable.

Conduct a Physical Examination
The physical exam is a head-to-toe examination that includes assessment of the lungs, abdomen, skin, musculoskeletal system, and more.

Electronic Color Vision Test

Includes distance and near vision, color vision, and peripheral vision. Applicants must meet 20/20 vision standards, with or without correction. There is an additional $60 charge for this test and it is required of individuals obtaining certification for the first time.

Hearing Test

You must be able to hear a normal spoken conversation and pass audiometric testing if required.

Blood Pressure Check

Blood pressure must be within FAA-acceptable limits.

Cardiovascular Assessment

Required for pilots over age 40 on an annual basis, including an Electrocardiogram (EKG) to screen for heart health. A single baseline EKG is required at the first exam performed after age 35. There is an additional $80 charge for this exam. 

Neurological Exam

Evaluates motor skills, reflexes, coordination, and mental status. 

Evaluate Additional Requirements
Depending on your age or specific medical conditions, additional evaluations or documentation may be required. I’ll guide you through these steps to ensure compliance with FAA standards.

First-time applicants have a couple of additional requirements. You'll need to complete an Electronic Color Vision Test to assess distance and near vision, color vision, and peripheral vision, meeting 20/20 standards with or without correction. This test carries an additional $60 fee. If you're over 35, you'll also need a baseline Cardiovascular Assessment, including an EKG to evaluate heart health. This exam is $80 and becomes an annual requirement once you're over 40.


Complete FAA Certification
Once your exam is complete and all requirements are met, I’ll issue your FAA Class II medical certificate. In most cases, the process is completed during your visit.
